The perceived sensitive nature and legal ramifications of EHRs often limit access, typically focusing the cohorts within on patients from a single hospital or network, thereby failing to capture the diversity of the broader population of patients. HealthGen, a groundbreaking approach to synthetic EHR generation, is presented here, capturing true patient attributes, temporal aspects, and missing information. We empirically validate that HealthGen generates synthetic patient populations which are strikingly similar to real EHRs, exceeding the performance of current leading approaches, and that the integration of synthetic, conditionally-generated cohorts of underrepresented patient groups into existing real-world datasets significantly elevates the models' ability to generalize across different patient populations. To improve generalizability of inferences from longitudinal healthcare datasets to underrepresented populations, synthetic electronic health records conditionally generated could prove helpful in increasing accessibility.

Transitioning digital health interventions from randomized controlled trials (RCTs) to routine medical center (MC) practice is a major challenge. This paper details a two-wave (2wT) scale-up method, comparing the safety and efficiency outcomes of the MC interventions. For the 5084 2wT men, 630 (124%) were supported by telehealth reassurance, wound care reminders, and hygiene advice through 2wT; further, 64 (197%) were referred for care, and half of these referrals resulted in visits. Just as RCT outcomes indicated, routine 2wT proved both safe and provided a substantial efficiency advantage over the in-person follow-up model. 2wT's implementation decreased the need for unnecessary patient-provider contact to enhance COVID-19 infection prevention. The sluggish pace of MC guideline revisions, combined with provider reluctance and inadequate rural network coverage, hindered the progress of 2wT expansion. Despite potential impediments, the rapid 2wT gains for MC programs and the potential positive effects of 2wT-based telehealth on other healthcare situations significantly outweigh any limitations.

The 2020 HSE report revealed that roughly 2,440 workers per 100,000 in the UK suffered from work-related stress, depression, or anxiety, resulting in an estimated loss of 179 million working days. To evaluate the influence of tailored digital health interventions in the workplace on employee mental health, presenteeism, and absenteeism, a systematic review of randomized controlled trials (RCTs) was undertaken. To locate RCTs, a comprehensive examination of multiple databases was undertaken, focusing on publications from 2000 forward. Data were meticulously entered into a standardized data extraction format. The Cochrane Risk of Bias tool was used to assess the quality of the research studies included in the analysis. We investigated the clinical paths taken by patients with breathlessness, employing process mining and its associated techniques. Our investigation of the literature employed a dual approach, focusing on clinical pathways for breathlessness as a symptom, and on pathways for respiratory and cardiovascular diseases which are commonly intertwined with breathlessness. The primary search encompassed PubMed, IEEE Xplore, and ACM Digital Library. We incorporated studies exhibiting breathlessness or a related illness alongside a process mining concept. Our review excluded any publications written in languages other than English, and those that prioritized biomarkers, investigations, prognostic factors, or disease progression over detailed analysis of symptoms. Prior to the full-text review, articles qualifying as eligible underwent a screening stage. From an initial 1400 identified studies, a total of 1332 were removed during the screening and duplicate removal stages. From a full-text review encompassing 68 studies, 13 were selected for qualitative synthesis. Within this selection, 2 (15%) were symptom-oriented, and 11 (85%) were disease-focused. While the methodologies across the studies varied considerably, just one incorporated true process mining, using multiple approaches to analyze the clinical paths in the Emergency Department. While most included studies underwent training and internal validation using single-center data, this limited their ability to be generalized. In contrast to disease-specific strategies, our review emphasizes the absence of extensive clinical pathway analyses regarding breathlessness as a symptom. While process mining shows promise in this field, its widespread adoption has been hampered by difficulties in data compatibility.
